Who we are

What we collect

When you send the consultation form, we receive what you type: your first and last name, your phone number and your email address. The form also records whether you checked the box agreeing to calls and text messages.

So we know which ads bring people to us, the page also records how you got here: the ad or campaign you clicked (the campaign, keyword and click identifier the advertising platform adds to the link), the page you arrived on, the website that referred you, your device type, the date you first visited, how many times you have visited, and which procedure pages you looked at on this site. Where Google or Meta has already set a cookie in your browser, its identifier is recorded too. This is stored in your own browser for 90 days and reaches us only when you submit the form.

We do not collect medical information on these pages. Anything you tell us at your consultation becomes part of your patient record and is covered by our Notice of Privacy Practices instead.

How we use it

To respond to your request: to call you, to text you if you agreed to texts, to answer your questions, to find a time for your consultation, to remind you about it and to follow up afterwards. Calls may be placed by a member of our team or by our automated phone assistant.

To understand our advertising: to see which ads and keywords lead to consultations, and to tell the advertising platform that showed you the ad that a consultation was requested, using the click identifier it attached to your visit. We do not send your name, phone number or email address to advertising platforms.

We do not sell your information, and we do not give it to anyone to market their own products or services to you.

Who handles it for us

Your details are stored in the practice's customer relationship system, run for us by a software provider under a contract that limits what it may do with the data. These pages use Google Tag Manager to run Google Ads conversion tracking so we can measure our ads; it receives the click identifier and the fact that a form was sent, not what you typed.

Text messages

If you check the box on the form, or say yes when we ask on a call, Schneider Centre for Plastic Surgery may send you text messages about your consultation request: replies to your questions, scheduling, reminders, follow-up after your visit, and secure links to forms we ask you to complete. Message frequency varies. Message and data rates may apply. Reply STOP at any time to stop receiving texts, or HELP for help. The full messaging terms are on our terms page.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

How long we keep it

We keep your contact details and the record of your request for as long as we need them to respond to you and to keep our record of who asked for a consultation. If you become a patient, the details become part of your patient record and are kept as the law requires.

Your choices

You can stop text messages at any time by replying STOP. You can ask us not to call you, or to correct or delete the details you gave us, by calling 239.277.9999 or writing to the address below. We will do so unless the law requires us to keep a record.

Changes to this policy

When we change what these pages collect or how we use it, we update this page and the date at the top.
